Public Water Supply District No. 1 of Sullivan County, Missouri is requesting proposals for services to complete a lead service line inventory. Seven copies of a proposal, including price for the services, must be mailed to and received at 1108 S. Pearl St., Milan, MO 63556 by Thursday, Feb. 15, 2024 at 2:30 p.m. local time. Late, conditional or electronic proposals will not be considered. Proposals must be sealed and clearly labeled "Lead Service Line Inventory", with the entity’s name and address on the outside of the envelope. Proposals will be read in public, and the selection may be considered at a regular Board Meeting.
This project is financed through the Missouri State Revolving Fund, by the Water and Wastewater Loan Revolving Fund and federal Capitalization Grants to Missouri.
Request for Proposals
Public Water Supply District No. 1 is requesting proposals for services to complete a lead service line inventory. The completed service lead line service line inventory spreadsheet should adhere to 40 CFR 141.84. The district has an estimated 2,850 service lines.
The district is requesting pricing for several services as part of this project: records review services, in-field verification services and administrative services. Bidders must provide a price on the below proposal form for each service requested. Proposals that fail to provide a price for all services will not be considered. Bidders should be aware that they will be expected to complete forms required by the state funding agency. Additionally, there will be language requirements and forms from the state funding agency that will need to be incorporated into the final contract that is signed by the district and selected bidder.
Proposals shall address, at a minimum, the criteria specified on the proposal requirements. Failure to include all information requested may result in the proposal being considered non-responsive. Public Water Supply District No. 1 of Sullivan County reserves the right to accept or reject any or all proposals for any reason.
Proposers are informed that pursuant to Section 285.530, RSMo. as a condition of the award of any contract in excess of $5,000, the successful proposer shall, by sworn affidavit and provision of documentation, affirm its enrollment and documentation, affirm its enrollment and participation in a federal work authorization program with respect to the employees working in connection to the contracted services. Successful proposer shall also sign an affidavit affirming that it does not knowingly employ any person who is an unauthorized alien in connection to the contracted services.
Proposers are informed that the project is subject to the requirements of Section 292.675, RSMo., which requires all contractors or subcontractors doing work on the project to provide, and require its on-site employees to complete, a 10-hour course in construction safety and health approved by the Occupational Safety and Health Administration (OSHA) or a similar program approved by the Missouri Department of Labor and Industrial Relations that is at least as stringent as an approved OSHA program. If on-site employees have not previously completed such training, the training must be completed within 60 days of the date work on the project commences. On-site employees found on the worksite without documentation of the required training shall have 20 days to produce such documentation.
Proposers for this work will be required to comply with the President's Executive Order Number 11246. Requirements for proposers under this order are further explained at the link provided below.
