Filing a Paper Report
To file a paper report, the generator or facility completes the applicable PDF fill-in form above and submits it to the department address on the form. The PDF fill-in forms may be saved and reused; however, you will need Adobe Acrobat Reader 7 or higher to save the form. Also, at this time the department does not accept digital signatures. If your Acrobat Reader prompts you to use one, we ask that you do not.
Filing Online Reports
The Generator’s Hazardous Waste Summary Report and the Facility Summary Report my be filed online through the department’s Missouri Gateway for Environmental Management (MoGEM) system. Through this system, a user is able to access multiple sites with a single account. It also allows multiple levels of access. A person can be registered as a Viewer, Preparer, Certifier or Organizational Official. Certifiers and Organizational Officials are required to have a valid e-Signature agreement to be able to submit reports. The Organizational Official is a Certifier that is able to manage the other system users for that site. The department does maintain Excel files that can be used to create files that allow generators and facilities to upload their report to MoGEM instead of completing the report entry on the website. This option should only be used if the generator or facility has large amounts of data and will be importing data from their computer system into the Excel file data sheet. If you need the Excel version of the report form to create files to upload to MoGEM, please send an email request to hazwastereport@dnr.mo.gov. The department will email the files to you.
Biennial Report
The Biennial Report is a federally required report for permitted hazardous waste treatment, storage and disposal facilities, large quantity generators and any other generator that opted to handle their hazardous waste as a hazardous secondary material. The report covers every odd-numbered calendar year and is due by March 1 of the following even-numbered year. Generators and facilities required to file a Biennial Report, complete and submit the report through the U.S. Environmental Protection Agency's RCRAInfo system.
